General Details

Login Details

The website URL where the theme will be installed.

I understand that most host logins require 2-factor authentication. If possible, please add hello@heartenmade.com as a user or contributor on the account. You can google "how to add user to [name of your host] account" to see how. If not, we can schedule a time for you to allow access if needed.

Please follow this new user guide to create a temporary Admin user for me. Paste in the password you created here.

Installation Details + Preferences

Your selection should match the "Site Type" that you chose when you purchased the Theme Installation.
The coming soon page will simply display a "coming soon" message with no other content.

Theme Setup Preferences

Header

Name of your blog or business.

If you prefer to use your own logo instead of the default text site title, upload it here. PNG or JPG format preferred.

The icon that displays in the browser tab. Image must be at least 512x512 pixels. PNG or JPG format preferred.

Navigation Menus

Please list the links you'd like included in your navigation menus, including drop-down items. If a page doesn't exist yet, I'll create a blank placeholder so I can add it to your menu — but you will be responsible for adding the page content to it.

If you'd like to link to specific blog categories in your menu, please note which items should be categories so I can set them up properly. If you have a New Site, I will create the categories for you!

Note: Menu placement and available locations vary by theme. Take a look at your theme's demo site — it will show you what menus are available and where they're placed.

If your theme has a sidebar, I can set it up with your photo, a short bio, and your categories.

I'm happy to make small, reasonable sidebar accommodations (like rearranging items or adding a search bar or social icons if they don't already exist for example). Let me know what you want here!

Additional Installation Notes

You can use this space to share anything else you'd like me to know before I begin your installation. For example, notes about your hosting setup or login details, details about existing plugins or settings that might affect your setup, etc.

Confirmation & Agreement

Please carefully read and agree to the terms below before submitting your form.
  • You understand that your site will be set up to look just like the demo site of the theme you purchased, using the theme settings you specified in this form.
  • This service does not include future edits or revisions after completion.
  • This service does not include adding content to your pages or any type of theme customization.
  • You understand that it can take 2–3 business days for your form to be reviewed, and the installation turnaround time is typically 4–7 business days after your form is reviewed. (Weekends and major holidays are not included in business days.)
  • You understand that turnaround time also depends on communication — if I have questions or need additional info and responses take several days, that will delay your installation.
  • All communication for this service will be handled by email only.
  • There are no refunds for this service once work has begun. If you wish to cancel before work starts, please contact Carissa at hello@heartenmade.com.
